We are a Foundational Community Supports (FCS) Agency.

What that means is…

We provide services to individuals who are experiencing a number of complex health and risk factors.

Our team connects people to the resources they need—like health care, income, social supports—in order to get and keep housing and employment. Because living with a mental illness, a disability, addiction and a criminal record can make it hard for someone to find a job or a place to live.

So…what is…Foundational Community Support? (FCS)

Foundational Community Supports (FCS) provides supportive housing and supported employment services to our most vulnerable Medicaid beneficiaries. These services are designed to promote self-sufficiency and recovery by helping participants find and maintain stable housing and employment.

Supportive housing services help individuals get and keep community housing. 

Supported employment services help individuals with barriers to employment get and keep a job.

Who is eligible for the FCS program?

FCS is a targeted Medicaid benefit designed to meet the needs of individuals with significant barriers to finding stable housing and employment.

Participants must be Medicaid-eligible and meet certain age requirements, along with meeting specific medical risk factors including chronic homelessness, complex behavioral health and co-occurring substance use needs among others, as well as a disability or other long-term care needs.

FCS does not pay for rent, rental subsidies, wages, or wage enhancements.

Someone that will be a positive & supportive while assisting you in evaluating your individual journey and to start planning to live a better life.

That “Peer Support person” is also responsible for demonstrating and inspiring hope, trust, and a positive outlook while connecting you with a broad range of community-based services and, while assisting with finding a home and employment.

Supported Housing Services

Our services are covered by your insurance. So there is no cost to you.

This is a program intiated by the Washington State Health Authority through DHSH.

DHSH - provides medical markers to your health file based on the information you provide.

Then you contact our team….

Our team will check for eligibility, by you providing your Apple One or State of Washington Services Card provider number. You can fill in the info in the side bar here on this page. Or call us for a personal phone at 206-657-3611 for a interview as well.

There is an eligibility process that takes place, with a interview, & then an assessment is sent in by our team, then we waits 10 days to see if you have been approved to now be a client for us to support and love on.
